ntegrated behavioral health in primary care has been established following decades of literature as a valuable system change to improve care to patients, enhance population health, decrease cost, and bolster joy in work for healthcare professionals. Implementation of integrated care – with several models, clinical pathways, and perspectives – can take many forms, depending on the organizational and practice setting, patient population, and social and behavioral determinants of health within the local community. This session will review lessons learned from family physicians and behavioral health professionals with leadership experience in implementing integrated care programs within primary care. Perspectives, clinical workflow examples, and key models for behavioral health integration will be reviewed, along with reflections on the change management principles needed to sustain and spread meaningful change.
