In this presentation, we will share real-world examples of value-based care delivery – from “no-risk” quality improvement models to “high-risk” capitation models (and everything in between). Additional topics discussed will include quality incentives and Accountable Care Organization (ACO) models that are aimed at improving quality and reducing costs.

We will also discuss best practices, common mistakes, lessons learned, and solutions specific to the challenges faced in rural delivery models. We will also share practical methods to overcome the challenges of patient engagement, accountable population health data and provider adoption.

Finally, we will draw upon our experience working with providers, payers, and health systems to paint a picture of the future of value-based care in the U.S. and how it will impact rural healthcare delivery

Sarah Chouinard, MD

Sarah Chouinard, M.D. is the Chief Medical Officer at Main Street Health where her leadership expertise in tech-enabled high-quality rural primary care, value-based care, and pay-for-performance models supports Main Street’s mission of providing solutions to help rural Americans access the high-quality care that they deserve. Prior to joining Main Street, Dr. Chouinard was the Chief Medical Officer of Community Care of West Virginia for over 15 years. She has been recognized as an Outstanding Rural Health Practitioner from Marshall University Joan C. Edwards School of Medicine. Her experience also includes serving as National Faculty for quality improvement initiatives at the Center for Medicaid and Medicare Services. Prior to this, she also served as the Medical Director for West Virginia’s Public Employees Insurance Agency (PEIA) where she worked on value-based care initiatives. She is a co-founder of the Fayetteville Skate Park, a multi-faceted recreational facility promoting physical activity and wellness. For her efforts in promoting free, healthy activities for her community, Dr. Chouinard was recognized as her hometown’s “Citizen of the Year.” Outside of her professional endeavors, Dr. Chouinard is a yoga Instructor and loves to spend time outside.
