Jacob Valvis was a recipient of the ACOFP Foundation Initial Certification Grant.

Thank you to the ACOFP Foundation for the privilege of receiving an Initial Certification Grant to help cover the costs of my osteopathic in-person practical examination. This grant is appreciated both for its financial assistance, but also because of its impact on my commitment to community, facilitating my ongoing dedication and commitment to my patients as an osteopathic family medicine physician.

As we all know, the financial demands of medical training are substantial and cumulative, so support is always appreciated. This grant provides more than immediate relief from examination fees—it represents a meaningful investment and trust in my ability to deliver comprehensive, patient-centered care and to contribute to the education of future physicians committed to holistic medicine. As I transition from residency to attending practice, this support helps me build a stable foundation with fewer financial stressors during a pivotal time in my career and personal life. I plan to stay local in Delaware, practicing in Sussex County as part of Beebe Healthcare, serving a growing and underserved community. I am working to establish roots here for myself and my growing family, prioritizing accessibility, continuity of care, and personal well-being.

Why osteopathic family medicine? I truly believe in treating the whole person and equipping myself with every possible tool to serve my patients well. I want to confidently say I have exhausted every resource within my scope before referring a patient to a specialist—especially when specialists are increasingly difficult to access. My patients face significant barriers daily: limited transportation, inadequate insurance, financial constraints, language differences, and other social drivers of health. Every referral asks them to overcome these obstacles again.

Osteopathic manipulative medicine (OMM) offers an additional, evidence-based modality—beyond medications, procedures, and referrals—to address musculoskeletal complaints, pain management, headaches, and functional limitations directly in my office. If I can provide effective treatment myself, I reduce patient burden and improve the likelihood of timely, appropriate care.

Family medicine is the cornerstone of the American healthcare system. We are often the first point of contact, the consistent presence across a patient's lifespan, and the clinicians who see the full spectrum of human illness—from newborns to the elderly, from preventive care to complex chronic disease management, from mental health to acute injuries. It is both a privilege and a profound responsibility.

Beyond patient care, this grant facilitates my ability to further the field through education. I am committed to modeling comprehensive primary care that demonstrates what is possible when conventional medicine is combined with osteopathic principles. I want to inspire the next generation of physicians to see beyond the prescription pad, to use their hands as diagnostic and therapeutic instruments, and to view each patient as a whole person. By investing in my training now, this grant helps ensure I can pay that knowledge forward for years to come.

All in all, the osteopathic practical examination is a critical milestone in achieving board certification in osteopathic manipulative medicine, validating both technical proficiency and a commitment to the highest standards of practice. With your financial support, showing your trust and commitment to me, I am honored and deeply grateful for the ACOFP Foundation's support in making this goal achievable.
