The ACOFP Diversity, Equity and Inclusion (DEI) Award recognizes osteopathic family physicians who make significant contributions toward enhancing diversity, equity and inclusion within osteopathic family medicine education and practice. This award honors practicing osteopathic family physicians who have completed residency and have demonstrated behavior or led initiatives that foster these principles within diverse and underrepresented communities, through engagement, initiative and leadership. 

Application Checklist

For your application to be considered by the ACOFP Awards Committee, the following items must be submitted by September 15, 2022:

  • Completed application
  • Current CV
  • At least one letter of support from nominator, state society or institution
  • 500–650-word personal statement
  • Headshot suitable for publication

Personal Statement Requirements

The 500–650-word personal statement must document how you have engaged in activity and demonstrated leadership affecting at least two of the areas listed below in efforts to correct these discrepancies:

  • Advocacy and actions you have taken to support the advancement of underrepresented minorities or those from disadvantaged backgrounds (including women, socioeconomic-disadvantaged communities, the LGBTQI community and African American and/or Black, Hispanic/Latinx, Native American, Pacific Islander, Asian or Middle Eastern communities) within the osteopathic profession.
  • Advocacy efforts and actions you have taken toward reducing health disparities in your state or at a national level.
  • Leadership actions and activities you have engaged in, while serving as a mentor for underrepresented students at a college of osteopathic medicine that advanced the professionalism of the student and encouraged or supported the student’s efforts to pursue training as an osteopathic family medicine resident.
  • Documentation and demonstrations of initiatives, actions, advocacy efforts, etc. you have supported or led that served to promote health equity and inclusion within the communities you serve, the osteopathic profession, a family medicine resident program or college of osteopathic medicine.
  • Actions you have taken or areas in which you have provided leadership to increase social accountability and ethical medical care and decrease health disparities through the education and expanded understanding of diverse and underrepresented populations, including patients, physicians, healthcare professionals, residents, students and the public. 

Award Criteria

  • Must be an active practicing physician member of ACOFP (not open to resident and student members).
  • Availability and willingness to present a recorded or live a MED Talk or serve as a discussion group or symposium leader on their efforts.
  • Availability and willingness to write a blog entry for acofp.net on one or more components of this broad subject. 
  • Willingness to cooperate with and appear in ACOFP and professional publications, including being highlighted in the ACOFP newsletter, Leading the Osteopathic Way.
  • Serve as advisor and counsel for the ACOFP Health & Wellness Committee when requested.

Timeline

  • September 15, 2022: application deadline
  • March 29 – April 2, 2023: awards presentation
Nominations are solicited from the general membership, with the selection made by the ACOFP Awards Committee and final approval by the ACOFP Board of Governors. The award will be presented during the ACOFP 60th Annual Convention & Scientific Seminars, taking place March 29 – April 2, 2023, in Orlando, Florida. The award stipend amount is $2,500.
