The concept of a patient-centered medical home (PCMH) is receiving a lot of attention as an important foundation for changing the way primary care services are delivered.

Payors, purchasers and policymakers see PCMH as a way to achieve the "Triple Aim," - improving access and patient experiences, increasing the quality of care and moderating costs.

Nationwide, more than 90 commercial health plans, 42 state Medicaid programs, federal agencies and the military have adopted the PCMH model on various scales.
