| ACOFP Family Physician of the Year |
The ACOFP Family Physician of the Year Award is presented to a member who exemplifies the ideals of the practice of osteopathic family physicians. Recipients have made outstanding contributions to the osteopathic profession and served their local communities through service organizations, scouting, PTA’s, etc. The individual should be a role model to peers and students. Criteria include ACOFP membership, active in osteopathic family practice for at least the past 15 years, and nomination by the ACOFP or AOA state society.

| ACOFP Lifetime Achievement Award |
The ACOFP Lifetime Achievement Award recognizes an outstanding individual who demonstrates career-long service to patients, to the profession of osteopathic family medicine, and to the American College of Osteopathic Family Physicians. Nominees are solicited from recommendations of ACOFP Board members, who provide a concise written summary of the nominee’s credentials. The Board may choose to not present the award in a particular year.

The ACOFP Distinguished Service Award may be presented to a professional or non-professional who has demonstrated outstanding service to the ACOFP.

| ACOFP Young Physician of the Year Award |
Named in Honor of Michael F. Avallone, DO, FACOFP dist.
The ACOFP Young Physician of the Year Award is presented according to the following criteria: completion of an AOA-approved family practice residency program; certification by the American Osteopathic Board of Family Physicians, active in osteopathic family practice for two to ten years; continuous membership in the ACOFP and in the ACOFP state chapter, if available; service at the national or state level in any leadership capacity; and demonstration of exemplary leadership qualities that would honor the memory of Michael F. Avallone, DO, FACOFP dist., after whom the Award is named.

| Namey/Burnett Preventive Medicine Writing Award |
The Annual Namey/Burnett
Preventive Medicine Writing Award competition is to honor the best preventive
medicine papers submitted by Osteopathic medical students, interns or
residents and fellows. The First Prize winner will receive $500 award,
plaque of recognition, round trip airfare to the ACOFP Annual Convention
& Exhibition, three nights lodging at the Convention Headquarters Hotel,
and the winner also will present their paper as part of the Public Health and
Wellness Committee Lecture. The Second and Third Prize winners will receive
$250 award, plaque of recognition, and will also have their paper read at the
ACOFP Annual Convention & Exhibition.
All papers must be submitted using the ACOFP

| ACOFP State Society Honors |
ACOFP State Society of the Year Award
This award recognizes ACOFP State Societies that provide regular and effective communications (an annual communications plan, newsletters, educational promotions, annual meeting promotions, an annual report, press releases, and other materials) with members and prospective members, including students and residents, as well as the general public. Three awards are available, one for each ACOFP size category, excluding students & residents:
• Large (300+ members)
• Medium (100-300 members)
• Small (under100 members)
ACOFP Excellence in Leadership Award
This award honors the year's most outstanding demonstration by an ACOFP State Society of leadership, creativity, and initiative in building membership or programming. The award is designed both to recognize achievement and to promote the sharing of ideas among ACOFP State Societies. Three awards are available, one for each state society size category.
ACOFP State Society Public Service Award
This award recognizes ACOFP State Societies for their public service endeavors. Such examples as projects which demonstrate innovative problem solving or address a need in the community, make use of existing resources and can be used as a model for replication by others are all good candidates for the award. Only one such award is available each year.
ACOFP State Society Member Service Award
This award honors those ACOFP State Societies who have accomplished many of the key objectives outlined above. The award encourages ACOFP State Societies to perform specific activities designed to increase public awareness of the importance of the Osteopathic Family Physician and increase benefits to their members. One or more awards will be available.

| ACOFP Student Chapter Honors |
President’s Award
Awarded to the chapter that best exemplifies the spirit and the goals of the Student Association of the ACOFP. This award looks at all chapter aspects/activities to determine which chapter has best represented the opportunities and benefits of: practicing as an osteopathic family physician; entering an osteopathic family medicine residency program and continued involvement in ACOFP through educational programs, lecture series, procedural clinics, membership recruitment, “strongest link” residents involvement, board review, health fairs, etc.
A. T. Still Award
Awarded to the chapter that best promotes the integration of Osteopathic Principles and Practice of our profession’s founder, A. T. Still, and their integration into family medicine. This award will be based on chapter sponsorship of events, speakers and/or workshops that address the utilization of OMT as a family physician.
Humanitarian Award
Awarded to the chapter that has actively participated in humanitarian events within their school and community and beyond, acting as an advocate for osteopathic physicians. This could include community health fairs, blood drives, public health initiatives, fundraisers for non-profit groups, etc.
Communications Award
Awarded to the chapter that has done the best job at keeping all student members, faculty, staff, and SAACOFP “in the loop”. Ideally, this chapter will have a system in place for communicating not only with those students on campus, but also with those on clinical rotations. Submission of e-mails, posters, announcements, etc. to be evaluated will be required.
