Evaluations & Reports for Residency Training
- Advanced Standing Request

The CBE must be kept at the residency site. If the residency program closes, all CBEs must be forwarded to the ACOFP.

The Core Competency Evaluation form is to be kept at the residency program for each resident, and must be available to the on-site inspector. Do not send the document to the ACOFP, AOA , or the OPTI.

All Residency Programs are required to submit Final Residents Reports for Residents that will complete their program in 2011. Each Program Director must submit the report.
