The ACOFP’s new 8-session Blueprints for Building a Medical Home webinar series will provide you with a step-by-step methodology for achieving Level 1 NCQA recognition at a minimum, whether or not you use an electronic medical records system at this time.

Who Should Participate 

The Blueprints webinar series is designed for participation by osteopathic family physicians and their office staff who are on the health care quality team and whose goal is to provide quality care while making a difference in the lives of their patients. 

By becoming a recognized Medical Home, your practice may be eligible for increased reimbursement that is available through participating private and public health care plans.*

Webinar Format

Each monthly one-hour webinar will begin at 8:30 pm Eastern and include live audio presentation with slides, followed by a question-and-answer session with the speaker.  One hour of AOA Category 1-A Continuing Medical Education credit is anticipated.

Select individual sessions at a registration fee of $25 per session, or register for the entire series for the reduced registration fee of $150.

Session A

Introduction to the Medical Home 

 

Session Description:  This session will provide an overview to the PCMH certification program and the rationale behind its use.  In addition, we will discuss how to prepare your practice staff, your own clinical workflows and processes for certification.

Session B

Standard 1 – Enhance Access & Continuity 

 

Session Description:  This session will discuss how to provide patients with increased access to care and information.  Cultural and linguistic considerations will be presented, as well as the importance of continuity of care.

Session C

Standard 2 – Identify & Manage Patient Populations 

 

Session Description:  This session will discuss how to collect demographic and clinical data for management of specific groups of patients (with high-risk conditions such as diabetes, asthma, or CAD).  In addition, this session will demonstrate how to pro-actively identify patients in need of preventative screenings such as mammograms, colonoscopies, etc. as well as routine tests for certain conditions (i.e. HgA1c in diabetic patients or bone density measurements in postmenopausal women).

Session D

Standard 3 – Plan & Manage CareSpeaker:  Algirdas A. Juocys, DO 

 

Session Description:  This session will address how the practice identifies patients with specific conditions, including high-risk or complex care needs and conditions related to health behaviors, mental health or substance abuse problems. Pre-visit planning, assessment of patient progress toward treatment goals, and identification (and solution of) patient barriers to treatment goals will be covered.  Medication reconciliation and electronic prescribing will also be discussed.

 

 

 

Thursday, March 8, 2012 – Speaker:  Thomas G. Zimmerman, DO, FACOFP 

Session E

Standard 6 – Measure & Improve Performance 

 

Session Description:  This session will discuss how the practice must use performance and patient experience data (such as rate of hospitalizations and ER visits) to continuously improve the quality of care and provider performance using both EMR as well as paper-based systems.

 

 

 

Thursday, April 12, 2012 – Speaker:  Thomas. G. Zimmerman, DO, FACOFP 

Session F

Standard 5 – Track & Coordinate Care 

 

Session Description:  This session will address how the practice tracks, follows-up, and coordinates tests, referrals, and care at other facilities (such as hospitals, physical therapy, chemo, etc.).  In particular, the use of both electronic medical records (as well as paper-based systems) to achieve these requirements will be discussed.

 

 

 

Thursday, May 10, 2012 – Speaker:  Joseph C. Mazzola, DO, FAAFP 

Session G

Session Description:  During this session it will be demonstrated how to assesses patient/family self-management abilities, and how to work with patient/family to develop a self-care plan.  Methods and strategies on how to connect patients with educational materials and community resources will also be discussed.

Session H

Session Description:  This session will summarize what has been covered in the previous sessions as well as provide guidance on the how to prepare and submit your application to become a recognized Patient-Centered Medical Home.

*ACOFP cannot guarantee that your practice will be approved to receive this reimbursement, nor can ACOFP guarantee that your practice will achieve Medical Home recognition. 

About the Speakers

Carol L. Henwood, DO, FACOFP is a member of the ACOFP Board of Governors and ACOFP’s representative to the Patient-Centered Primary Care Collaborative (PCPCC).  She is the Chair of the Quality Committee for Pottstown Medical Specialists, Inc., a primary care multispecialty, physician-owned practice in Stowe, Pennsylvania that has achieved Level III status as a NCQA-recognized patient-centered medical home.

Algirdas A. Juocys, DO is the Chief Medical Officer of Medical Network One, a Michigan-based physicians’ organization and is in private practice at Country Creek Family Physicians, a designated BC/BS Medical Home in Rochester, Michigan.  He is a member of the ACOFP Practice Management Committee.

Joseph C. Mazzola, DO, FAAFP is Vice President and Director of Medical Education for Blue Ridge HealthCare in Morganton, North Carolina where he is working on obtaining medical home certification for two residency programs as well as 12 primary care practices within the health systems medical group.

Thomas G. Zimmerman, DO, FACOFP is Director of Medical Education and Family Practice Program Director at South Nassau Community Hospital in Oceanside, New York.  He is a member of the ACOFP’s Board Task Force on Patient Centered Medical Home.  His residency clinic was recently certified by the NCQA as a Level II patient-centered medical home.
